I need a small preamp, in the range of 6db. The problem is with a distributed av system (runs sound C5) and the oudio output from the directv STBs is substantially lower than the other sources. The MCA-C5 allows for 3db of adjustment but I am still not fully driving the amps. yes, I already went into the STBs and set the audio out to the highest setting

Radio Design Labs (RDL) makes stuff like that. Not cheap new, but if you can find one on eBay, they're dirt cheap. I sold 25 assorted RDL modules for $41 last year. I sat on them for 10+ years, and never used one.

I see they make a balanced version too, do you know if they make a RCA version? In the past I have used an "audio buddy" it's just that with the 1/4" inputs and weird jack / knob layout they are awkward to tuck away in the back of a rack, and in this case I need 3 of them..

Nope, only screw terminal. that's the way all RDL stuff is.
