Pretty sure you are supposed to do this while you are making the MKV...although I haven't had many playback devices actually detect the chapters. I load the chapter file in mkvtoolnix and it never seems to work in the end. I will also be curious to see if you can add them somehow after the fact...

Seems like there is a pointer missing that tells the player's firmware where the chapter information is located in the file sector. Of course, I know nothing of MKV and only guessing here with disk analogy. Hah!

Chapter do work in MKVs with MPC-HC on a PC. My GUI put them in, but it re-encodes the audio to FLAC, I don't think a PS3 will play FLAC audio.
I also don't know if chapters are supported on a PS3.
You might check out Clown-BD.
