Electrical Equipment 101 - Open Ground Fault

Hi All --- Yet another electrical problem in my system, which likely involves a solution that is obvious to those with a rudimentary understanding of electrical equipment, but which has confounded me.

I just installed into my system a Furman PS-8R power sequencer that I am using to enable a remote power up of my amplifiers, two of which do not have remote power up options. The Furman unit needed repair when I bought it and I managed to repair it and install it in a manner that will enable a 12v signal from my Lexicon surround processor to initiate the power up sequence on my amps.

The Furman is downstream from a medical grade isolation transformer, and I moved all the power connections for my amps, surround processor, satellite receiver, DVD player, and video processor to the Furman. (I have my Electrohome 9500 on a separate circuit, which I like will change soon to obtain whatever isolation/power conditioning benefit is available.) The Furman has a set of indicators on the front that now indicate an open ground fault. This indicator was not present when I tested the Furman separately before connecting it to my equipment.

How can I identify the source of and correct this problem? Help would be appreciated.

Is it indicating a fault on the line in or in one of the outs? Are all the amps, etc. grounded? Try unplugging them one at a time to see if it is one of them causing the fault.
